Someone rejects you. A relationship ends. A friend excludes you. You don't get the job. Someone you love chooses someone else.
You're not physically injured. Yet your chest feels heavy. Your stomach tightens. Your body aches. You may even describe it as pain.
That's not just a metaphor. Your brain treats social rejection as a genuine threat.
Your Brain Is Wired for Connection
For most of human evolution, belonging wasn't optional.
Being excluded could mean losing protection, resources and even survival.
So the brain evolved systems to detect social disconnection.
When rejection occurs, the Salience Network—which detects what is important, threatening or emotionally significant—can engage in ways that overlap with the emotional component of physical pain.
Research suggests social rejection and physical pain share some neural mechanisms, particularly those involved in the unpleasantness of pain.
But rejection does not simply switch on the same "pain centre" as a broken bone. The two experiences also have distinct neural representations.
The brain isn't confusing heartbreak with a broken arm.

Why Rejection Can Feel So Physical
The Salience Network detects what matters, while the Interoceptive Network monitors what's happening inside your body—your heartbeat, breathing and stomach.
Together with networks involved in emotion and regulation, they can turn rejection into a powerful whole-body experience.
So when someone says, "I feel sick after the breakup," they're not necessarily being dramatic.
The brain is changing the body's state.
And Then the Brain Replays It
Physical pain usually fades when an injury heals.
Social pain can return months later.
A photograph. A song. A familiar place. An unexpected encounter.
The event is over.
But the brain can reconstruct the experience, reactivating neural systems associated with affective pain.
Memory can become a trigger for pain without a new injury.

Why Some People Hurt More
Rejection doesn't affect every brain equally.
Self-esteem, attachment patterns, previous experiences and how we interpret social information can influence the intensity of the response.

Rejection hurts because connection matters.

That's why a breakup can produce physical sensations, exclusion can consume our attention, and an old rejection can suddenly hurt again.
